Self-Retracting Lifelines Recall, click the Submit a Claim button to begin your shipping reimbursement claim. You will need the following items to submit your shipping reimbursement claim:
Case Number/Claim Number listed in the Subject line of the email notification you received from 3M confirming submission of your recall claim.
Number of Affected Units.
Total Amount of Reimbursement Requested, including Value Added Tax (VAT) and other duties and fees.
Receipts/Invoices from your Shipping Carrier.
VAT Statement (if applicable).
Wire Instructions.
You must submit a separate claim for each Case Number you received. Do not combine reimbursement amounts from multiple Case Numbers into a single claim.
